Request Electronic Door Access

Submission Process

To gain access to a specific door, complete the 105-156 form, which serves as the official request document. Please note the following guidelines:

  1. One Door per Form: Each form allows access request to only one door. If access to all doors on a campus is needed, select "Campus Master."
  2. Card Reader Code: Identify the card reader code located on the top of the door's card reader. Include the first four or five numbers in your request.
  3. Approval Process:

    1. Requestor's Supervisor
    2. Dean (if applicable)
    3. Director of Campus Operations of campus where the door is located
    4. Additional approval required if the request is for after hours access (police approval required).
    5. Central NOVACard will receive notification after all required approvals are given and will grant access accordingly.
