Academic Integrity Forms

Academic integrity violations may be reported through the Academic Integrity Violation Reporting Form. When submitting a report, include relevant documentation and supporting information to assist the Office of Academic Integrity with the review process.

Report an Academic Integrity Violation

Students who wish to appeal an academic integrity decision may do so in accordance with Procedure 224P.

Students have two options for submitting a first-level appeal:

• Contact the appropriate Academic Dean directly via email, with a copy to the Office of Academic Integrity; or
• Submit the Academic Integrity Appeal Form. Submitted forms will be reviewed by the Office of Academic Integrity and forwarded to the appropriate Academic Dean or appellate authority for review.

If a student wishes to pursue a subsequent level of appeal following a first-level appeal decision, the student may contact their campus Provost directly via email and copy the Office of Academic Integrity, or submit the Academic Integrity Appeal Form indicating that they are requesting a second-level appeal, in accordance with the appeal procedures outlined in Procedure 224P.

Appeals must be submitted within the timeframe established in Procedure 224P. Students should refer to Procedure 224P for information regarding appeal requirements, grounds for appeal, timelines, and the complete appeal process.

Academic Integrity Appeal Form

• First-level appeals are reviewed by the appropriate Academic Dean or designated appellate authority. 
• Additional appeal review may be available through the Provost, as outlined in Procedure 224P.
• The Office of Academic Integrity assists with routing submitted appeal forms but does not make appeal decisions.
• Students should consult Procedure 224P for specific appeal grounds, requirements, and timelines.

Some situations may involve both academic integrity concerns and student conduct concerns. For example, allegations involving academic misconduct may also include conduct that violates the Student Code of Conduct, such as failure to comply with the directions of College officials, disruptive behavior, or other prohibited conduct. In these situations, the matter may be reviewed through both the Academic Integrity Process and the Student Code of Conduct Process.

Report a Student Conduct Concern

1. Report Review
   • The Office of Academic Integrity reviews the report and routes it to the appropriate instructor or College official.
2. Student Notification
   • The student is notified of the concern by the instructor or other designated College official and is provided an opportunity to discuss the incident, respond to the allegation, and provide additional information.
3. Review and Determination
   • The concern is reviewed by the instructor or other designated College official, and a determination is made based on the preponderance of the evidence standard.
4. Outcome and Sanctions
   • If responsibility is determined, appropriate sanctions may be assigned by the instructor or other designated College official in accordance with Procedure 224P.
5. Additional Review by OAI
   • Students with multiple findings of responsibility under Academic Integrity Policy 224 may be referred by the Office of Academic Integrity to the Office of Student Rights and Responsibilities for administrative sanctioning, in accordance with Procedure 224P.
6. Appeal Rights
   • Students have appeal rights as outlined in Procedure 224P.
