About the Forms Library

Welcome to NOVA's Forms Library, where you can access forms frequently used by the College. The forms are available in Rich Text Format (.rtf), Portable Document Format (.pdf), Online or Excel.

• Forms in .rtf format can be read and completed using Microsoft Word or any other word processor. They can also be saved and printed.
• Forms in .pdf format can be read and printed using Adobe Acrobat Reader.
• Online forms can be completed in your browser; click on the button and following the directions.
• Forms in Excel Spreadsheet format can be read and completed online using Microsoft Excel. These forms can be saved, edited and printed.

If you need a form that is not available online, please contact the office responsible for that specific form.
