» Make-up Tests
When a student misses one of your exams, you may place a make-up test for that student in the Testing Center. Generally, no more than 5% of a class should be permitted to take make-up tests for any one test administered in class. In exceptional cases, such as illness or out-of-town travel, with permission of the Director of the LRS or the Testing Center Supervisor, with a time frame of no less than a week to take the test, you may place exams for your entire class in the Testing Center.
All tests placed in the Testing Center should be properly labeled at the top along with any special instructions. Please be sure that the test identification coincides with the information on the test pass. Any information pertaining to taking the test should also be on the front of the "Request for Make-up Examination" form, which is provided by the Testing Center.
For administrative purposes, it is important that the staff receive exams that are to be taken in the Testing Center at least 24 hours prior to the scheduled testing time.

» Frequent users of the Testing Center
If you teach Developmental Math, PLS Math, or one of the courses, which offer a second unit test in the Testing Center, you should inform your students as to what will be expected of them in the center. Copies of a hand out which explains Testing Center procedures are available from the Testing Center for distribution to students.
NOTE: Because of the large volume of Development Math students, if the Testing Center is overly busy, especially with placement testing, the grading of math test may be discontinued at the discretion of the Testing Center Staff.
» Test
Pass:
All students taking an exam in the Testing Center
MUST have a test pass. The pass should include:
Instructor's name
Course name
Course number
Designated test (i.e. Unit 3, Test 4 or Ch Chapter 6)
Instructors should be sure the test pass is properly
filled out to ensure that the student receives the correct exam. If
you are unable to personally give the student a test pass, you should
be sure the student understands that the test pass should be picked
up at the Division Office and not at the Testing Center.
Test pass exceptions: If you are placing only a few
make-up tests in the Testing Center, write the student's name on each
test in lieu of providing that student with a test pass.
Under special circumstances, if you are placing tests
for an entire class in the Testing Center include a class roster with
the tests. With appropriate identification, we will allow those students
to take the tests. Please make sure those students know the instructor's
name, course name and number and designated test (i.e. Unit 3, Test
4, Chapter 6, etc.) If they do not, they will not be able to take the
test.

I.D.
Please inform students that they must have their picture
I.D. (driver's license or military I.D., or passport) with them when
they come to the Testing Center. If you know that one of your students
does not have a picture I.D., please note this on their exam and advise
the Testing Center to accept this student without a picture I.D.
