There are a lot of free Javascript quiz utilities out there.

Quiz Creator is from the SuperScripter column at Builder.com. It is an HTML-based form; supply it the numer of questions, what type of questions, and the content of your questions, and it will generate one HTML page with all questions coded and scored, with the answer key supplied at the end.
