| The Check Box | A check box is a form element
similar to a radio button except that it allows users to make more than one choice in the
offered list. The checkbox tag requires a VALUE for each possible choice. VALUE refers to the actual data being collected. The VALUE attribute identifies the information gathered when a user specifies a particular choice from a list provided by you. |

| To add a check box to your web page using
FrontPage 98, you can choose Insert, Form Field, Check Box from the main
menu. You can also use the Check Box button on the Forms toolbar. |

| To specify the properties of the
check box, highlight it. Choose Edit, Form Field Properties from the main menu or right click on the form element. The Check Box Properties dialog box will appear. |

| In the Name text box fill in a description of the information you are gathering. (In our example above, the name might be called status). | ||
| In the Value text
box fill in the name of the value for that particular button (In our example above the
value for the first check box would be fulltime). Click on OK. |

| Repeat the process for the
remainder of the check boxes in this section of the form. Choose Initial State Checked if you wish a particular check box choice to be pre-selected. |
