Create Check Boxes
| After you have created your form, you are ready
to design the form itself. Remember that you create a form field with the <FORM></FORM> tag pair. Within the <FORM> tags will go the entire form, i.e. both the questions you ask and the form elements, i., e., the box and button spaces where the user will supply responses. |

| The Check Box | A check box is a form element
similar to a radio button except that it allows users to make more than one choice in the
offered list. The checkbox tag requires a VALUE for each possible choice. VALUE refers to the actual data being collected. The VALUE attribute identifies the information gathered when a user specifies a particular choice from a list provided by you. |

| To add a check box to your form in HomeSite, choose the Form element button on the Advanced tab. The Form Element dialog box will appear. |

| In the Form Element dialog box choose Checkbox. The Form Element dialog box for a check box will appear. |

| In the Name text box fill in a description of the information you are gathering. (In our example above, the name might be called status). | ||
| In the Value text
box fill in the name of the value for that particular button (In our example above the
value for the first check box would be fulltime). Click on OK. |

| Repeat the process for the
remainder of the check boxes in this section of the form. If you wish a particular checkbox to be pre-selected, manually add the word CHECKED to that checkbox's <INPUT TYPE> tag. |
