FORMS Library
Forms are in .rtf and .pdf format. Some forms can be submitted online only.
The .rtf documents can be read and completed online using Microsoft Word or any other word processor (Note: Always click the field to fill out the content instead of using the Tab key). These forms can also be saved,edited and printed.
The .pdf documents can be read and printed online using Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader or would like to upgrade your current version, you may download Adobe Acrobat Reader on your home computer or request your campus IT Manager downloads it on your office computer for you. You can save and print this but you cannot edit it after you submit it. 
The Online form is completed online and submitted to a database. When the login screen appears after you click on the form, you MUST complete the information in the login screen to access the form.
a. Enter NVCC for the Domain Name (if requested)
b. Type nvcc\ (be sure you use a backslash \ and not a forward slash /) before your User name and then your User name (ex. nvcc\jdoe); next enter your password in the space provided. Your NOVA User name and Password are what you use to sign on to your office computer (It is not your NOVAConnect/SIS/AIS User name and password.)
