Students must request reader services four (4) weeks prior to each academic semester or once notice is given by instructor for an exam. We recommend that students who will need textbooks on tape contact their disability services counselor as soon as they have chosen their classes. This should be done at least four weeks in advance. Students are responsible for purchasing their own textbooks.

All classroom accommodations must be approved by the counselor. Students must submit a copy of the approved request for accommodations to the instructor

Students and counselors will visit classrooms to determine accessibility. Students may request reassignment after paying for courses if the classroom is inaccessible. Contact a Disability Services Counselor if this is necessary.

Equipment to be checked out must be a recommended accommodation and indicated on the Memorandum of Accommodation form provided by the Disability Services Office. Students and their counselors will agree upon a return date.
