Obtain an information packet from the
receptionist in the Counseling Center, CG 211 or you can print on
online.
Obtain copies of the documentation
of your disability. This documentation must be no more than three
years old. For returning adult students records may be dated no more
than five years. The documentation must be by someone such as a medical
doctor or licensed psychologist. The documentation must provide a
diagnosis, the current level of functioning and how the disability
currently impacts on the student's level of functioning. An IEP is
not sufficient documentation. Your records will be on file in
the counseling office and will not be part of your permanent college
record.
Schedule an appointment with a disability
services counselor. You may do so in person at the Counseling Center
or by calling 703-323-3200.
To expedite and facilitate the process,
we recommend that all forms be submitted to the counselor prior to
the appointment and no less than 30 days prior to registering for
classes.
In the event that an accommodation is denied by the
disability services counselor or by the professor, the student may
appeal the decision through the student grievance procedure as outlined
in The College Student Handbook.
